GR.1A XZ108/GD of 54 Sqn from Coltishall, Norfolk, while both flying at low-level on approach
to ranges at Spadeadam. The Tornado crashed into a field after the two crew had ejected safely and
the Jaguar managed to fly on to RAF Leeming for a safe landing.

Africa. Engine flameout at low level when aircraft passed through the wake turbulence of the lead
aircraft of a two ship low-level ACM training sortie, control was lost and the pilot was unable to
recover in the altitude available. He ejected and was later picked up by a 22 Sqn Puma from
Ysterplaat..

FEEDBACK 1	The 2 CF-18s that collided over Europe on 17 April 1990 were 188765 and 188779. Both were from No. 439 Squadron, 1st Canadian Air Division. Sorry, no information on fate of the crew.
FEEDBACK 2	Regarding the CF-18 mid-air crash on April 17th 1990 over Karlsruhe, Germany: The pilot of the second Hornet involved in the crash was Capt. Timothy Leuty (he flew 188765). He was killed during the crash, the cockpit section was smashed by Capt. Decoste's aircraft wing in the collision. Also, Capt. R. Decoste miraculously survived the landing with his parachute on a highway with heavy traffic. That highway was the so called "Süd-Tangente" (Southern tangent - as it runs from west to east along the southern portion of the city) in Karlsruhe, Germany. I haven't seen the crash with my own eyes, but I was just minutes later getting out of the car at the civil airfield of Karlsruhe (we had a flying club there) and a double-seater CF-18 was circling over the city for at least half an hour shortly after the crash. Perhaps they were trying to take photos from the aircrafts debris which was all over the place as the aircraft collided at an altitude of around 14000 feet, though no one was hurt on the ground.
FEEDBACK 3	*The pilot of the CF-18 Hornet that was killed over Karlsruhre West Germany on 17 April 1990 did not die because his parachute malfunctioned.* I was the WO (Warrant Officer) from CFB Baden on the Crash Recovery Team/Crew at this time. Capt Leuty was killed by the impact of the wing tip of Capt. Reg Decoste's CF-18 penetrating Capt Leuty's aircraft and cockpit. I know this because I talked to Capt Decoste and was debriefed by him. I also personally observed Capt Leuty's cockpit (or what remained of it) and it showed definite signs of massive trauma). West Germany. Both pilots ejected but one was killed when his parachute failed to open, the other

formation during exercise Elder Forest and subsequently lost all hydraulic power. Pilot ejected
safely and was picked up by an oil-rig supply vessel. Aircraft crashed into the sea off Spurn Point,
Humberside.

Air Force documents show that a $20 million F-16 that crashed near here on Dec. 1 went out of control after the pilot paused to urinate into a plastic pouch during a routine training flight.

The pilot, Capt. Craig Fisher, a veteran flight instructor, told crash investigators that he had been unable to regain control of the jet even when he switched the plane to its backup flight control system, according to the documents, which were obtained by The Los Angeles Daily News through a legally enforceable request under the Federal Freedom of Information Act.

Captain Fisher parachuted to safety after the jet had dropped from an altitude of 28,000 feet to 7,400 feet. The jet crashed near the Palmdale Air Terminal
