5th January 1989 10.00h	U.S. Air Force	F-16A block 15	83-1116	Homestead Air Force Base	Surface attack/tactics training mission. Hit by turkey buzzard at 515 knots. Canopy penetrated and struck pilot. Crashed southern end of Dade County in a cow pasture north of Lake Okeechobee located in south-central Florida.			ACES II

USAF Phantom F-4C 63-7626 of 123rd FIS/Oregon ANG from Portland, Oregon crashed in sea 30
nm off Tillamook Bay, Oregon. Crew ejected safely.

RAF Phantom FGR.2 XT908/CW of 228 OCU from Leuchars crashed into the North Sea 50 nm off
Dundee during a training flight. Crew ejected at 5,000ft. Pilot missing. Major injuries sustained by
the navigator. The aircraft was descending, wings level, at 250 kts.

RAF Tornado GR.1 ZD891 BB of 14 Squadron, from Bruggen, West Germany flew into a
formation of 8 West German Air Force Alpha Jets on approach to Wittmund colliding with two of
the trainers. The Tornado and one of the Alpha jets crashed near Weismoor. Two crew of Tornado
killed. Pilot of Alpha Jet 40+87 of JBG 43 ejected.

Two Libyan Air Force Mig 23 Floggers from Al Bunbah Air Base engaged two USN F.14
Tomcats from USS John Kennedy, flying CAP, 60 miles off the coast of Libya. Both MiGs were
shot down and two parachutes were reported, presumably ejected. Neither pilots were found
following extensive Libyan sea and air search.

Royal Netherlands Air Force GD F-16 Fighting Falcon crashed on landing
at Leeuwarden Air Base. Pilot ejected.

USAF GD F-16C Fighting Falcon 86-0297/WP of the 35th TFS from Kusan, Korea, crashed into
the South Korea Sea. Pilot ejected and was rescued by US Army Chinook.

US Air Force General Dynamics FB-111A 68-0243 380th BW crashed into woods north-east of
Vermont, both crew escaped (capsule ???)

USAF F-4E Phantom 68-0465 from Indiana ANG/122nd TFW/163rdTFS at Fort Wayne attempted
an emergency landing at Baer Field City Airport after the pilot was incapacitated in his smoke filled
cockpit. The WSO tried to land the aircraft but shortly before touchdown he ejected and the pilot
and the aircraft crashed. Pilot was killed and WSO seriously injured.

crashed in southern Ohio after engine failure. Pilot ejected safely.

whilst attempting a forced landing there due to technical problems.

Habton, North Yorkshire, after the pilot ejected safely.

Two USAFE GD F-16C's, 86-0311/TJ & 86-0312 TJ, from the 401st TFW/612nd TFS at Torrejon
collided in mid-air near Valverde del Jucar, Cuenca Province, Spain. One pilot ejected but the other
was killed.
