To update your info.  You have the right date October 4 1966 for my ejection.  I was with Capt. William Andrews.  Bill was killed by ground troops after we were on the ground for several hours.  I was rescued by Jolly Greens at around noon.  Bill's remains were returned to the U.S. in 1974 or 1975.

First launch with trim setting based on ca release. After launch, the a/c remained for 3 secs in the launch attitude, climbing slightly, it then started to rotate quickly, achieving a 20 degree nose up attitude. After a further 1 sec the a/c rolled to port. The aircrew ejected at 80 ft with 30 degree bank. Excessive nose up trim setting which was subsequently amended for further ca release. The a/c impacted the sea 14degrees 33.5 n, 119 degrees 40.5 e (off Subic Bay, Phillipines).
