Rick Hormel (only son of Hormel Ham) died in a
departure accident in the VA-122 in about 1974 over the Owens Valley
(tried to recover but did not quite make it.)

I might be wrong but I believe Jake Piet jumped out of a number of A-7’s, I want to say three.  I don’t see his name at all.  I don’t know how to verify that.  I am sure they would be interested in that stat.

Stain Curtain and Sundance Harrison both concur that Jake Piatt jumped out of three Corsairs during his colorful career... can you confirm or deny that, and if it is true, do you recall which squadrons and approximate time frames that would have happened?  For some reason, I did not know Jake... and if he jumped out of three airplanes, I probably was the only Corsair driver who did not know him...LOL

  Did anyone see or remember the mid-air with an east coast A-7 guy who was hit from behind by an F-4 during climb out. The F-4 guys did not make it but the A-7 guy whose name was "Gus" - I think - was OK. Believe the squadrons were VA-82 and VF-11. I was the CAG LSO with Spanky Graff.

 how bout the guy coming home to Cecil from a cross country to the West Coast who passed out or went to sleep, overflew NZC and continued out over the Atlantic... National Guard scrambled and rendezvoused to find him slumped over in the cockpit... probably hypoxia... mask off.

  There is one correction or maybe it's just more info, but it sounded like some one thought Pete Leum had flown into the ground.  That was not the case.  Pete had just left VA 105 for orders to China Lake Weapons and was on an early intro flight to the school in a TA-7C with an ensign in the backseat for a FAM ride.  On climb out, the engine exploded and took the tail of the aircraft off.  105 was told that they were doing 350+ when the engine let go.  They found panels from the top of the wing 1 mile from the crash sight.  The person who called 105 to let us know, said that both people were dead almost immediately because of the negative G's.  Just wasn't Pete's day!

I know of one other, it was from USS FORRESTAL, the pilot was Layne Smith, a nugget in VA-81(?) who has a PC-2 failure at night off the coast of Florida.  He blew his gear down, then had a run of bolters before hitting bingo fuel...dirty bingo, except they forgot to tell him to climb to 15K enroute.  He flamed out about 1/2 mile on final to NAS Mayport, FL, ejecting safely.  The jet landed in the surf break...saw it the next morning...kind of sad to see.

  Commodore John Waples sent me home on Cecil's C-1 to represent the Navy and the squadron as Dan Ryder and his Marauders were still deployed in Nimitz.  Hal disappeared while on a SSSC mission off of Nimitz near Sardinia. Assumed to have flown into the water.  No wreckage or body ever recovery.  No oil slick either... still a mystery

  VA-122...early 1969?... RAG aircraft on a low level Ground Mapping Spoiled/Terrain Following Radar hop with Thermal Shield down flying though valleys in Eastern California/West Nevada desert.  First tour pilot flew blindly into mountain.  Apparently was NORDO but had not realized it.  Chase Instructor Pilot called for the RP to CLIMB... but no response.  That hop was taken out of the syllabus following that mishap.  I flew it on my way thru VA-122, and it was a bit dicey, but a great confidence builder if you didn't buy the farm...or  in this case the side of a mountain...

Then there was the VA-174  TA-7, intentional gear-up landing at MCAS Yuma, AZ with Bill "Cowboy" Johnston and Bill "Beetle" Bailey, at the controls.  I happened to be the OIC of that Weapons Det.  Beetle and Cowboy were returning to land from a "spotter" mission in the TA-7 and had an unsafe gear indication.  After attempting to get the gear down by every means known to the entire Naval Aviation community, and after I discussed the situation with home plate at NZC on the telephone, these two sterling Naval Aviators, both of whom later became big-time airline pilots, continued to fly the two seater until they were virtually out of fuel, and then shot a flawless, gear-up, straight in landing to a foamed runway... shutting the engine down by securing the fuel control just as they crossed the numbers.   I have always regretted that these two were never recognized for their superb flying skills on that occasion... a DFC might have been in order, or at least Individual Air medals. 
 
As an aside, that TA-7 was later temporarily repaired at Yuma with the gear being locked in the down position and a piece of sheet metal mounted along the damaged portion of the belly.  I was dispatched to return to Yuma to bring the A/C back to NZC for futher repairs.  Terry Miller accompanied me to be my chase in one of the Weps Det planes.
 
We flew the entire hop with  my plane's gear down; Terry used partial flaps.  Made two refueling stops along the way... flew at 19,500' and at 210 kts (below gear speed).  Got back to NZC just in time for a passing thunderstorm... we both earned our flight pay that day....

There was an ejection at Whitehouse, in the early 70’s, Cant remember the guys name and it was a weird circumstance,

I want to say wings folded or tried to trap or something like that. E survived the ejection but I think he was boarded out.

I remember a JO jumping out of a jet in the Cecil Landing pattern, the Jet almost landed at Herlong field. This would have y been mid 80’s.

I know there is more that I will think of.

Vought test pilot John Omvig was doing touch and goes and on the last one the #1 A-7 began to roll inverted and he ejected just before it rolled 90 deg. Those that saw it from closer up said he was about the height of a telephone pole when the chute deployed. The A-7 impacted on the golf course which was about 3 miles (straight line) SE of the approach end of the primary runway From Dick Atkins, Archives Director of the Vought Aircraft Heritage Museum, the cause was pilot error when the hydraulic system was switched off (flight test configuration) and loss of control resulted. John ejected and barely made it out alive. He was later killed in the XC-142A V/STOL  aircraft crash in May 1967.

  The U.S. Navy's first A-7 loss occurred on 22 December 1967, less than three weeks after entering combat. The Corsair from VA-147, USS Ranger, was piloted by LCDR James M. Hickerson, was attacking a AAA site when a SAM exploded under his aircraft, his engine and hydraulics failed and he ejected. LCDR Hickerson was released as a POW on 14 March 1973.

I flew on the 10 June 1968 RESCAP of LCDR Randy Ford. My logbook entry is dated 11 June but it was a middle of the night mission. Early-released POW Mark Gartley reported that Randy died from complications of a severely broken arm while in captivity before he was transported to Hanoi. Randy was shot down near Vinh.

LCDR David "Scott" Grieling apparently died in his crash on the night of 24 July 1968. Reports that he was a POW were later proven to be inaccurate.  A few years ago, a team found his crash site and saw that the ejection seat was still in the wreckage.  His 'dog tags' were reported to have been seen by an Eastern European sailor (I believe in a bar near Haiphong) but there is no evidence Scotty survived the crash.

The 'ejection' on 2 Sept 1968 was an Air Wing staff officer who accidentally flew into the water on a night visual approach. I'm not positive but I believe his seat fired accidentally as he was cart-wheeling after impact. Most of his injuries were from him skimming the water after ejection. He survived that night but was killed a few years later in a mid-air collision over the bombing target at Vieques, Puerto Rico. I knew him and will recall his name eventually. I think it was Bob but right now I can't come up with it.

 

Also, the name of the pilot who ejected on 2 Sept 1968 after striking the water while on approach to the USS America was Don Bennett. I think he was a LCDR at the time and was the Air Wing LSO flying an A-7A from VA-86.

Charlie Cates, was killed during an ejection at Guam during some training flights. He blew a tire, was running off the runway, headed for an obstacle of some kind, decided to eject and started just as the airplane impacted. From that ejection we got the "breaker bars on the seat rather than having the canopy leave before the seat rocket fired.

In 1970 a Lt. Watkins, instructor in 174 jumped out of a jet on approach to Cecil in a Thunderstorm, I believe it was also an engine failure.  The amazing thing about this one was the aircraft ended up vertical in the pine trees, nose down good shape.

Ltjg John Pianetta, on an Instrument Bombing Derby from Lemoore to Fallon.  Thermal Shield down.  Following a level delivery in an A7-E at 10,000', he started to raise the thermal shield to go visual per the chase pilot's instructions and was inadvertently ejected over the EW range.  The aircraft, I believe NH-400, in attitude hold, continued fly  pilotless northward with enough fuel to make it into Canada... I had just taken over as the Dambuster's Safety Officer when the call came in from Fallon that the aircraft had "Crashed" and that the pilot was safely on the ground.... shortly thereafter, it was discovered that the a/c had not crashed and was still airborne.  We went to GQ as the potential was there to become an international incident if it made it into Canada. I quickly brushed up on how to send a "Flash" msg to the White House...LOL 
Because it was in attitude hold, as it passed over various mountain ridges, the a/c was buffeted and did a slight right wing down turn to the right, disappearing in the shadow of a mountain near Provo,Utah where it was assumed it had crashed when it did not reappear to Oakland Center.  A western rescue outfit initiated a search in that area, and even had witness on the mountain who claimed the a/c passed just overhead at a very low, tree-top level... so it must have crashed into the mountain.  The search went on for many days... no crash site. The squadron got a new, replacement aircraft and continued our turnaround training.  When we finally deployed in Kitty Hawk months later the a/c still had not been found; so we went to war with it.

Many months later after an extended deployment we returned to NZC, and shortly thereafter the aircraft was discovered.  It had run out of fuel and landed itself virtually intact in a level mesa... can't remember if it was in Utah or Idaho... I was the lucky Safety Officer who got to write the mishap report for Paul McCarthy. 

Our investigation found a design flaw in the non-telescoping canopy release handle in the cockpit that positioned the pilots elbow immediately above the handle when his hands reached for the thermal shield handles.  Based on the distance on the ground between where the canopy was recovered and where John landed, it was determined that John inadvertently cracked the canopy when his elbow hit the release handle, the wind took it off, the thermal shield imploded on his helmet, pulled the face curtain out of its detente and into the wind, which subsequently came out far enough to fire the seat.  As a result, that apparatus was resigned and refitted on all A-7 aircraft. . 

Later, Circa 1973, [
7th February 1973] just after I departed for Bupers and the Dambusters were training for yet another deployment, that same John Pianetta was the lead of a nite two plane radar round-robin when his first-tour nugget wingman suddenly disappeared from radar and John's wing during a level turn south at altitude over Alameda. 

That plane went straight down into the roof of an apartment building; miraculously, no one was killed except the pilot... most all of the residents were at a nearby carnival and out of the building. . . "

An article appeared in the November 15, 1972 edition of Nhan Dan newspaper  in Hanoi which stated that the Vietnamese forces had shot down a U.S.  aircraft on that date and that the pilot ejected but the parachute did not  open

"I was a member of the Blue Angels maintenance team from January 1967 to September 1969.  During the 1968 show season my pilot was Lt. C.O. "Smokey" Tolbert.  He flew number 6 jet that year. At the end of the season he transferred to a fleet squadron flying the A-7 aircraft. On November 6, 1972 while on a mission over VietNam he was shot down and never recovered."

Low ceilings and reduced visibility necessitated individual attacks into the  heavily defended target area. On making individual runs into the target area  the number three aircraft saw a parachute floating down through the clouds  and heard an emergency radio beeper just prior to bomb release. A momentary  emergency radio transmission was heard saying, "I'm in the water with two  A-6s passing overhead". This was followed by 2-3 unintelligible  transmissions and then then one-way radio contact was lost.

There was an ejection at Whitehouse, in the early 70’s, Cant remember the guys name and it was a weird circumstance, I want to say wings folded or tried to trap or something like that. E survived the ejection but I think he was boarded out.

I remember a JO jumping out of a jet in the Cecil Landing pattern, the Jet almost landed at Herlong field. This would have y been mid 80’s.

I know there is more that I will think of.

had an aft tank transfer failure in an A-7E, which caused a thermal disconnect and then the flameout.  He ejected safely at 4000' on the starboard side of USS CORAL SEA, CVA-43.  The helo was there just about the time he splashed.  It was quick and became one of a couple of Aft Tank Transfer Failures, which resulted in "the switch" finally being installed in the aircraft to select motive flow!

My father, CDR (then Lt.) Dennis "Smiley" Reilly survived an ejection from a VA-86 Sidewinders A-7E, BuNo
159659. He was at NAS Roosevelt Roads, Puerto Rico, at the time. Date was 28 August 1984. He had to punch out because of a hydraulic failure and loss of control. His ejection was sucessful, he landed in a swamp and was picked up by a passing Navy truck. The a/c landed in 40 feet of water near the runways, all wreckage was recovered.
(We still have pieces of it at our home).

Hey, I was in Puerto Rico when he did that!
I was working the fuel farms.
The story I heard was that the gyro internals were installed wrong at the local AIMD and that when the pilot engaged the AFCS it caused the aircraft to go out of control.
Out AE's had to pull all of the flight control gyros from our EA-7L's so they could be inspected.

I watched the A-7 take off and launch from the runway. When he was airborne sucked up the gear and flaps.
I remember that the aircraft started to yaw from side to side while still
flying in a straight line. It then started to pitch up and down, and finally it went almost straight
up.
That's about when the pilot ejected. I can still see the black smoke from the engine at full power when the plane
went almost straight nose down behind the commissary. There was an explosion, followed by a large white cloud of smoke and a
geyser of water, and then black smoke.

They pulled the plane out of the water, piece by piece. The biggest piece was the what was left of the engine casing and one of the UHT's. Everyone thought that the plane had crashed into the commissary parking lot.

Tell your dad it looked "cool" from where I was, although me and the other guys who were working the fuel pits were screaming "EJECT-EJECT-EJECT" after the first couple of side-swapping yaws!

I was trying to get the jet to Pinecastle before jumping out. 
I didn't make it to PC, I ejected over some lake and the jet crashed in Buck Lake.  That was 29 November.....and the water was COLD!!!!!!

Lt Pohlmeyer was actually in VA22  in May of 1986.  He was a twin,  and his brother was also in VA22.  Very sad day for all of us.   We called them "The Pohlmeyer Brothers".  (Of course)
 
The other pilot was VA122,   I believe.
 
I was a "I" Level Avionics Fire Control Technician specializing in the A7 HUD (or PDU) and  ASCU (Armament Station Control Unit).  Served 1983 -1987

*Among his survivors is a twin brother, Paul, a pilot for Lemoore-based Attack Squadron 127.

 
Neither of the fliers aboard the A-7E Corsair IIsout of Lemoore Naval Air Station was immediately identified. The light attack jets carried no other crewmen, a Navy spokesman said.

Taking part in the search were the 210-foot Coast Guard cutter Resolute, two Coast Guard helicopters from Humboldt Bay, a fixed-wing Falcon jet from Sacramento and a small boat.
